A Clinical Analysis of Periampullary Cancer


Abstract
Sine periampullary cancer often presents late clinical manifestation and difficult anatomical location, the correct preoperative diagnosis is difficult. Despite recent advance in medical knowledge and surgical techiques, the prognosis is poor.
Author evaluated 68 patients who received operation for periampullary cancer in the Dapartment of Surgery at Pusan National University Hospital, from January 1986 to December 1991.
@ES The results were as follows:
@EN 1) Among 68 cases, 38 cases(55.9%) were pancreatic head cancer, 18 cases(26.5%) were distal CBD cancer, 12 cases(17.6%) were Ampulla of Vater cancer.
2) The male and female ratio was 1.5; 1. The prevalent age was 6th decade(32.4%), and mean age was 51.9-year-old.
3) The major clinical symptoms and signs were abdominal pain(66.2%), anorexia and indigenstion(64.7%), jaundice(52.9%), weight loss(47.1%), fever and chilling(33.8%)
4) Important laboratory findings were increased alkaline phosphatate(73.5%), bilirubin(66.2%), AST and ALT(61.7%).
5) The valuable diagnostic methods were CT, PTC, ERCT and Ultrasonography.
6) Adenocarcinoma was the most common type(97.1%).
7) Pancreatoduodenectomy was performed in 38 cases(55.9%), total pancreatectomy in 2 cases(2.9%), bypass procedures in 22 cases(32.4%) and biopsy only in6cases(8.8%).
8) Postoperative complications were wound infection(13.2%), pulmonary complications, pancreatic and bile leakage, and bleeding etc.
